INFORMATION TO HELP YOU REGISTER A DEATH Documents required by the Registrar.

• Medical Certificate of the cause of death.
• Birth Certificate.*
• Medical Card.*

* If lost do not worry, provided you know the answers to the questions here.

Who can Register the Death.

• The Executor.
• A close relative or civil partner of the deceased.
• A relative or civil partner in attendance during the last illness.
• A relative or civil partner living in the district where the death took place.
• A person present at death.
• The person causing disposal (NOT THE FUNERAL DIRECTOR)
