Information on Registering a Death

Who Can Register The Death?

  1. The Executor.
  2. A relative or civil partner of the deceased.
  3. A relative or civil partner in attendance during the last illness.
  4. A relative or civil partner living in the district where the death took place.
  5. A person present at death.
  6. The person causing disposal, ie the person responsible for payment of the funeral account. (NOT THE FUNERAL DIRECTOR)

Documents Required By The Registrar

  1. Medical Certificate of the cause of death.
  2. Medical Card... If available

 Information Required By The Registrar

  1. The date and place of death
  2. The full name & surname (and the maiden surname if the deceased was a woman who married).
  3. The date and place of birth.
  4. The occupation (and if the deceased was a married woman or a widow the name and occupation of her husband).
  5. The usual address.
  6. Whether the deceased was in receipt of a pension or allowance from Public funds.
  7. If the deceased was married the date of birth of the surviving widow or widower.
  8. Name of informant, qualification and usual address.

Documents Issued By The Registrar.

  1. Certificate of Burial or Cremation (GREEN in colour) to be handed to the Funeral Director. Issued free. Not issued when there is a Coroner Post Mortem and funeral is a cremation.
  2. Certificate of Registration or Notification of Death D.W.P. notification. Issued free.
  3. Copy Death Certificates These presently cost £3.50 and are required by Banks, Insurance e.t.c.
