Information on Registering a Death
Who Can Register The Death?
- The Executor.
- A relative or civil partner of the deceased.
- A relative or civil partner in attendance during the last illness.
- A relative or civil partner living in the district where the death took place.
- A person present at death.
- The person causing disposal, ie the person responsible for payment of the funeral account. (NOT THE FUNERAL DIRECTOR)
Documents Required By The Registrar
- Medical Certificate of the cause of death.
- Medical Card... If available
Information Required By The Registrar
- The date and place of death
- The full name & surname (and the maiden surname if the deceased was a woman who married).
- The date and place of birth.
- The occupation (and if the deceased was a married woman or a widow the name and occupation of her husband).
- The usual address.
- Whether the deceased was in receipt of a pension or allowance from Public funds.
- If the deceased was married the date of birth of the surviving widow or widower.
- Name of informant, qualification and usual address.
Documents Issued By The Registrar.
- Certificate of Burial or Cremation (GREEN in colour) to be handed to the Funeral Director. Issued free. Not issued when there is a Coroner Post Mortem and funeral is a cremation.
- Certificate of Registration or Notification of Death D.W.P. notification. Issued free.
- Copy Death Certificates These presently cost £3.50 and are required by Banks, Insurance e.t.c.
